This medication chart/log helps you to record which medications have been taken and when, which can come in handy when you have numerous medications to keep track of.
There are two different styles-the first is more like a traditional drug chart with 4 boxes to fill in for each drug: medication name, dose, why it's taken and route (oral, inhaler etc). Then simply circle the time it's due or write a custom time in the column before the date columns start. See a whole month's medication administration on one sheet.
The second layout allows you to simply list medications as they are given.
