Medical Appointment Notes

This appointment diary and notes set features 3 different templates. The first is a year-view appointment calendar which allows you to keep track of multiple different appointments in one place. The second template is a monthly-view which is helpful if you have numerous appointments per month. It enables you to record the most important information for each appointment, keeping all this information on one page. For more in-depth note keeping there is template 3. The appointment notes page allows you to record in depth information to keep on top of your health care. Record the date/time/address to make it easier to find in a hurry, the relevant symptoms/medical condition and any discussion points you'd like to bring up at the appointment. There are boxes to complete for the plan going forwards, follow-up appointments/scans/tests and a notes and to-do section.
