This diabetes journal helps you to keep track of your general health/medications/appointments and of course your blood sugars. The first page is a handy medical overview of your health-medical history, surgical history, medications, religious beliefs, smoking history etc which can be very useful at medical appointments/hospital visits. The journal features a whole day overview of meals, blood sugars and medications given allowing you to keep track of how the foods you eat affect your blood sugars and how much medication is given. There is also a simplified week view blood sugar chart if you'd rather not record meals. The appointments page is a helpful reminder of upcoming appointments and contact details for if you need to re-schedule. There is a page to record any foot ulcers/wounds (for people with peripheral neuropathy) to remind you to check your feet and to help monitor any wounds that might need medical attention. The medications page allows you to record your allergies and current medications including any previous medications you've tried and why they were stopped. The last page features an action plan box, following a discussion with your doctor you can fill this in so you know what to do if your blood sugars are too high or too low. There's also a handy notes and to-do section along with local pharmacy information.
