This blood pressure planner helps you to keep track of not only your blood pressure, but also your general health, medications and appointments.

The first page is a handy medical overview of your health-medical history, surgical history, medications, religious beliefs, smoking history etc which can be very useful at medical appointments/hospital visits.

The coloured blood pressure chart is modelled on what is used in hospitals today-the different colours give you an idea of where your BP and chart rate falls in terms of normal ranges-if it falls in the red range you may need to seek urgent medical help. There is a weekly and monthly chart to keep track of home readings.

The appointments page is a helpful reminder of upcoming appointments and contact details for if you need to re-schedule.

The medications page allows you to record your allergies and current medications including any previous medications you've tried and why they were stopped.

The last page features an action plan box, following a discussion with your doctor you can fill this in so you know what to do if your BP is too high. There's also a handy notes and to-do section along with local pharmacy information.
